If your child requires medication to be given during the school day please speak to the school office before downloading and completing the form below and returning it to school.
Healthcare Plan
If your child has an on-going medical condition that requires on-going medication e.g. asthma, nut allergy etc. please speak to a member of staff as soon as you become aware of their condition and complete the Administration of Medication form and the Healthcare plan below in school with a member of staff.
